Change your laundry detergent to all-free & clear. Don't use any creams or lotions on your chest. Sometimes cotton shirts can cause this, particularly if you run or work out. This is known as "jogger's nipples.".

Very sensitive skin and most common cause is dryness.
Try over the counter Hydrocortisone cream for a couple of weeks. If the itching persists, particularly if it is on only on one side, see a dermatologist.
