In brief: Abn sperm, Diabetes

Diabetes like any serious medical problem can affect the sperm.
There can be other reasons and the diabetes may not be the problem. I like to think about sperm function and not just count. Some men with low count can have normal function. You need a comprehensive evaluation by a fertility specialist for male infertility (check doc credentials). Treatments are successful even if icsi needed.

In brief: ? varicocele

Your sperm count might be low if semen was collected soon after previous ejaculations, ideally collection should be after at least three days after last ejaculation.
Poor motility could be related to a varicocele. You can self examine and feel your left scrotum whist standing up, varicoceles feel like "a bag of worms", disappear when lying down. ? Consider ivf if sperm count remains poor.

In brief: IVF with ICSI

The best treatment for low sperm counts and motility is in vitro fertilization with intracytoplasmic sperm injection.
With this procedure the sperm count will be very adequate.

In brief: Consider IVF with IC

Best and perhaps only option for a good chance of pregnancy is ivf with icsi.
