Sharp pains can indicate infection of the nipple or ductal system, sometimes fungal. Over the counter topical fungal cream is worth trying, but if symptoms don't improve or redness develops, you'll want to see your doctor. Chest wall tenderness is usually musculoskeletal and Ibuprofen will help.
