In brief: Thumb in abduction
Dequervains involves the abductor pollicis longs and extensor pollicis brevis tendon slips which pass through the first dorsal compartment (tunnel) at the wrist.
Only the thumb and wrist need be immobilized, not the index through small finger. Usually the wrist is immobilized in very slight extension (bent back) with the thumb in a slightly extended/abducted position (off to the side).

In brief: The most comfortable
The metal strip is there simply to support the thumb so the tendon can rest.
The key is comfortable support and rest for the thumb. The exact position is less important. Be sure the brace/splint and the metal strip are not somehow creating new pains.
