Pvcs/PACs worse on some days. Never chest pain dizziness or SOB. When do you advise one go to ER for ectopics? 2 echos both normal mild TR,PR,MR.

If the extrasystoles occur in long runs or there is an episode of paroxsysmol tachycardia going to the ER is advised.
Otherwise for frequent symptomatic Pac's and Pvc's outpatient treatment by a cardiologist is reasonable even in the absence of structural heart disease. A trial on beta blockers could be effective.
