In brief: Yes

If you have a rash that is not responding to current treatment it would be wise to have it evaluated by a healthcare provider. If the rash is ring worm (fungus) then it may take a while to respond to topical anti-fungal cream treatment.
It may take weeks to fully resolve but should at least start to improve and stop spreading early in treatment (about 7-10 days).

In brief: Ring worm

If you have been using an antifungal cream on a skin lesion and the lesion has not resolved, you may not have ring worm. I recommend you visit your local dermatologist for a different diagnosis and not go to the emergency room.

In brief: ? emergency ?

What about a simple skin condition would put your needs into the same class as car crashes, heart attacks, broken bones, etc.
? Emergency rooms are not for ringworm. A prime reason for your treatment to fail is the diagnosis is wrong. Have this evaluated during regular hours at a drop in clinic or your doc.
