Apparently, you have been lucky with no symptoms from the incidentally found left hydronephrosis over the past 20 years.
Clinically, doing nothing with a watch and see would be the most reasonable thing to do after a set of evaluation with history+urinalysis+blood test for kidney function and renal US as base line info. Meanwhile, avoid vigorous contact sports so to minimize injury to such kidney.
