In brief: Pinched nerve

Radiating pain to the front or down the leg is usually pressure on a nerve coming out of the spine.
There are many causes and most are from disc disease, spurs, or degenerative diseases. However it's not impossible to have cancer cause this same problem. If this has been going on at least two weeks and anti-inflammatory medications are not helping then see a doctor for the right kind of x-rays.

In brief: Joint arthritis

Without leg pain involvement, back pain, referring into the front lower abdomen, groin, buttock or hip/thigh, may be coming from the arthritic joints of the lumbar spine.
Tlak to your doc about meds to help that problem. If that doesn't work, you might need pt, and/or injections of those painful joints (facet) of the back.
