Infection is the biggest risk.
Bleeding and swelling can occur leading to trouble urinating. Most are done in urologist office while awake and there can sometimes be pain. Few patients are done under anesthesia based on patients health. Transrectal is how most biopsies are accomplished.

Prostate biopsies are very well tolerated.
They are done in the office transrectally under ultrasound guidance. Usually 12 cores are taken after numbing soluton is injected. Risk of serious infection is 1/1000. Antibiotic pills are usuaully given before and after the biopsy.Bleeding from the urethra happens but is self limited and temporary. Blood in the ejaculate is for up to 6 weeks.
