I think digoxin therapy should only be considered in patients who continue to have signs and symptoms in spite of proven available other therapies, such as b-blockers, acei/arb's and diuretics.
The other area where digoxin may be helpful is af rate control although recent negative data in regards to mortality raise some concerns.
