Tested not treated? You were tested for a number of things, but were you ever treated for bacterial vaginitis? It would be worth an empiric trail of treatment if no retained vaginal object such as a tampon is found.

Probably still yeast. If you had typical symptoms of yeast infection (vaginal/vulvar irritation, scant discharge, etc) and the same thing is happening now, then yeast is still the best bet. It's often difficult to confirm yeast by exam and testing. It would make sense to try an OTC yeast treatment (oral Diflucan® or a vaginal cream with an "azole" active ingredient).
