In brief: Always

Any new lumps, bumps, skin changes in the breast should be evaluated by your doctor. Great job doing your breast exam and finding it.
Most breast masses are benign but it is very important to have it evaluated.

In brief: See your MD

Yes, you should see a physician for an exam and probably need a mammogram.

In brief: Ultrasound

If you are 19 then i would certainly not recommend a mammogram, there is no evidence that it is indicated in young women such a yourself.
The test you want to ask for is a breast ultrasound. And yes, see your doctor anytime you notice a new mass that doesn't go away.
