Anal spasm. Anal or levator muscle spasm itself doesn't cause stool leakage, but associated anal fissure, lax sphincter tone or mild inflammation of the rectum(proctitis) may cause leakage of rectal mucus. A proctologist or colorectal surgeon would be best qualified to evaluate and treat you.

Increase Fiber. This sounds terrible for you. Chronic constipation can result in anal pain and spasm. Yes S3,possibly s2. Sacral nerves 3 & 4, & sometimes 2, combine to form the pudental nerve which innervated the anal sphincter. S1 is not involved, but if S3 and possibly S2 are compromised, then this would yield sphincter dysfunction as you describe. Treatment options should be discussed with an orthopedist or neurosurgeon. Is there a medication my gastro could prescribe that might relax the internal anal sphincter to make the passing of stool easier? My stool is soft.

My anal sphincter feels like it will not relax properly on one side when evacuating stool. Plan to see GI doc, but what test/imaging can check this?

Many tests. It sounds like u have functional defecation disorder, or FDD. As it is disorder of function, ur best test is anorectal manometry: a pressure-sensing catheter is placed in the rectum & records pressures while you try to evacuate stool.
