Sounds like an infected cyst.
These cysts are actually fairly common in the tissue behind the ear. If it is bothering him it can be removed fairly simply with a procedure done under local anesthesia.

It most likely is an infected sebaceous cyst. But this is a benign inward growth of skin and often becomes contaminated with bacteria and thus the fetid (bad) odor. If it is large, it ought to be removed as Dr. Astrachan suggests.
