How long does it take a perianal abscess to drain? I had one lanced 4 days back but still feel the abscess build up with every bowel movement.

In brief: 10 days

On the average (I've seen 400 of these), while it is healing, it still has an inflammatory mass for 10 days.
You will feel this fullness for about 10 days, sometimes less. It should be soft by 10 days.

Most ER physicians (not surgeons) make too small of an incision for correct drainage. But if appropriately done, I stand by my suggestion. Most need at least a 2-3 cm lance.
