Likely BFS. If you have had this for years with no weakness then benign fasciculations syndrome is highest on differential. Make sure you had your magnesium levels checked. in certain cases magnesium supplementation helps. cut back on your caffeine intake and decrease your stress. consider eval with EMG testing to be sure it is just BFS; if it was ALS,MMN,myopathy etc you would have developed weakness by now.
