In brief: Visible prominences.
Bone spurs around a joint may show a visible bump and have less motion.
A heel spur us only visible on an x-ray but really isn't very important.

In brief: Depends
Bone spurs can occur almost anywhere.
The most common site i see in practice is in the heels. However, spurs occur in the spine and around any joint. It can be painful or asymptomatic. Many times the spur is not the cause of the pain such as heel spurs. The surrounding soft tissue becomes inflammed and painful.
