In brief: Infection, Stone, CA

There are many reasons for blood in the urine.
The most common is a urine infection. Other causes include bladder or kidney stones, bladder polyps, interstitial cystitis, and of course bladder cancer. Other sources of blood include the kidneys and ureters from disease or cancer. Kidney problems need to be worked up for "glomerulonephritis" and cancer.

In brief: Blood in urine

Most common reason is cystitis-infection of bladder causing break down of small vessels releasing the blood cells seen in urine.
However other benign and malignant conditions may produce same symptoms requiring further investigations like cystoscopy.

In brief: Many problems

There are several possible causes of blood in the urine.
Most of the time this is due to an infection, but it could also signal bladder or kidney stones, tumors, cancers, polyps, or unknown reasons. In general it is best to be seen by a physician to determine the cause as continued blood loss can lead to anemia, infections, or more serious problems.

In brief: Several reasons

Depending on the age group, the reason may vary.
It can be caused from kidney or bladder stones, infection, scarring, trauma, or cancer. Any blood in the urine needs to be evaluated by a urologist.

Sometimes what looks like blood ISN'T blood, by a by product of muscle enzymes called myoglobin, or cpk. This can be from using bodybuilding supplements, excessive exercise with not enough hydration and overheating. If so, it can cause tea colored urine that some might interpret as blood. See you doctor ASAP, it may be important for your kidney health.
