Generally speaking, muscle weakness is something a patient perceives and something that we can test for in physical examination by doing a manual muscle test, etc.
Atrophy is something we observe or visualize. In some cases they can be caused by the same thing (ex. nerve injury, immobilization, joint trauma), sometimes they are separate issues.
