PCOS should not have any long-term effect on the ovary, however there is an increased risk of endometrial cancer in women who have polycystic ovaries? This is due to the irregular menstrual cycle's and lack of progesterone exposure to the endometrial lining.
That is why it is so important that women have regular cycles, and are often on birth control pills long term to give them exposure to progesterone and reduce their risk of endometrial cancer.
