My creatinine level jumped from 1.05 in march to 1.4/1.5 declining to 3.2 due to AKI/CHF and now at 2.3. Do I need to worry about dialysis now?

Unfortunately your kidney function has declined considerably.
Please be seen by a nephrologist. You may be getting too much diuretic treatment. You should be on statin, beta blocker, ACE inhibitor and spirinolactone under medical supervision. Limitation of protein intake to 1gm/kg of body weight, limiting salt to 2 gm per day and fluid to no more than 60 ounces a day will be beneficial.
