Sperm counts are affected by body heat.
The testes like temperatures around 95 degrees and therefore the use of boxer underwear, not briefs allow the testicles to "hang loose" and optimize sperm production. Also, use only water based lubricants during intercourse and not oil based like vaseline.

An examination by a fertility trained urologist and/or a reproductive endocrinologist are advised before you change to a different type of underwear. It is not likely that simply changing underwear will make a difference.

Unfortunately, testing results are very limited. There is evidence that common anti-hypertensive medications that are calcium channel blockers may reduce fertilization in men who use them to control blood pressure. Check with your physician to see if other medications could pose problems.

A multivitamin with antioxidants can be helpful, consider resveratrol. Enzyme coq10 may enhance sperm motility especially for men who are taking statins for cholesterol problems. Carnitine supplementation with proxeed may enhance sperm motility. Also, stop the hot tub, steam room or sauna.
