In brief: Could be hernia.

A hernia could exist without a discernible bulge.
The other symptoms are consistent with a hernia. Could also be kidney stone, an issue with the sigmoid colon, pelvic lymph node enlargement...in short, you need some testing. Blood work and a CT scan of the abdomen and pelvis would be good places to start, along with a good physical exam.
