In brief: Safe, however...

... you should only take with regular monitoring by your doctor. Janumet (sitagliptin and metformin) is a combo-drug formed by adding sitagliptin (aka, Januvia) and metformin, hence the combo brand name (and combo price tag).
It has a BLACK BOX WARNING for potential lactic acidosis which can occur due to metformin accumulation. This is rare, but fatal in 50% of cases, esp. in diabetics w/ kidney disease. Talk w/ your doctor.
