Treatment for a first episode is the most critical and reduces the risk of neurological complications, limits the severity and duration, and relieves symptoms. The recommended dose of Acyclovir for a first episode of herpes virus cold sore is 200 mg orally five times daily for 5 days. Three times a day is not recommend as the drug lasts only 2.5 to 3.3 hrs in the system. Do not stop the drug.

When the blisters form, the virus is actively replicating and can infect others. So, it's important to avoid touching the blisters and to wash hands. Avoid touching the eyes. Acyclovir will only shorten the cold sore but it is not a cure unfortunately. Relapses occur more than 1/3.
