The reason is due to the gas in the stomach that is usually there interfering with the sound waves of the ultrasound. One might consider a CT to evaluate the right upper abdominal area where the bowel is or other tissues ultrasound might not have seen

An ultrasound can miss gallbladder disease and ,if there is a high index of suspicion, a HIDA scan is a much better test and shows function.
