In brief: Anatomy

You could be having symptoms secondary to a disc problem over the lower back.
An MRI study could reveal extent of problem. Physical therapy might help, yet if there is chronic and substantial nerve root compression, a surgery might be indicated.

Family Medicine

In brief: Sciatica. See FP doc

Common causes of sciatica in your age group include herniated disc, spodylolisthesis and piriformis syndrome.
An evaluation by your family physician should be sufficient to determine the etiology and he/she may order imaging (xray/MRI) to help with the diagnosis. Treatments depend on etiology and include steroid injections, PT, and medication specific for nerve pain.
