In brief: Ectopic

Ectopic pregnancies are emergency situations and must be treated when identified.
The only was to confirm this is by a combination of presentation (pain, bleeding, etc) laboratory studies (beta HCG levels) and by ultrasound examination to confirm the presence of a gestation in the uterus or outside.

No. You need an ultrasound and blood work.
If you have bleeding combined with pain (especially on one side of the pelvis) and you have a positive pregnancy test, you should seek medical help quickly.

No, blood work is very important.
Even if a ultrasound is negative and your bloodwork shows a positive beta hcg, a ectopic can not be excluded unless a intrauterine gestation is seen. So serial bloodwork and sonograms may be needed.
