Needs to be maintained above 550 for at least 6 months before one would expect to see any recovery from the sensory ataxia (balance issues) I recommend use of methcyanocobalmine or sublingual B12.

reversal of white matter changes highly unlikely

Reversal of WM changes is difficult to be sure but CT assessment of such degeneration is insensitive at best compared to MR or even clinical measures of dysfunction. Secondly, even if white matter changes could be anatomically precisely verified- exact correlation with those changes and a person's functionality is not well understood nor even predictable based upon best radiographic data.
