From a vascular standpoint this may relate to poor arterial flow as a result of "clogged" arteries supplying the area, similar to other types of pad. Smoking is also a very common cause and most urologist recommend avoidance of tobacco prior to intercourse. There are multiple other causes and a urology evaluation may be helpful.
