In brief: Neuropsych evaluatio

you are taking a lot of medications and obviously have a complex disorder. Several of the medicaitons can cause the difficulty focusing, attending and idistractibility characteristic of ADHD.
Before you add Adderall, (dextroamphetamine and racemic amphetamine) you should have an assessment of the medications you already take for necessity and efficacy.
