"Saggy eyelids" can mean so many things and the degree of "sagginess" is extremely varied.
Mildly wrinkled eyelids can be very well treated with cosmetics. gentle peels, fillers and lasers but severe laxity, large protruding fat deposits and eyelids that hang off the eyeball at rest (ectropion) are probably best treated surgically.
