1st, why do you think it's yeast? 2nd, where is it? 3rd, does he have any other medical issues that have to be addressed together? Perianal/rectal itching may not be yeast.
It's best to get an exam with KOH slide. He can tolerate the topical vaginal cream (similar formulation for jock itch/tinea cruris) over all genitals. He could try suppository in rectum at own (low) risk of mucosal irritation.

More often than not, yeast infection is a clinical diagnosis, and KOH prep is not always essential
