What is a complex breast cyst, and why would an aspiration, and core needle biopsy be recommended?

To rule out cancer. A complex cyst means it is not a simple sac filled with liquid (like a water balloon). Think of a water balloon filled with some smaller water balloons, pebbles, and dirt. This can hide cancer, which is why you need a biopsy. Make sense?

Yes. If the MRI is negative the likelihood of cancer is less than 2%. If an MRI is not obtained, a follow up ultrasound examination should be obtained and, if the cyst recurs, a core needle biopsy of it may be indicated. I assume you have had a mammogram? There's no size. Size cannot tell a doctor whether a cyst needs to be biopsied. A 1 cm breast cyst could be totally benign, or it could be suspicious for cancer. The same is true for a 5cm cyst.
