There are many causes of shoulder pain, some requiring surgery, some not. Xrays cannot evaluate tendons, ligaments, bursa, labrum, or muscles, so a normal xray doesn't rule out an abnormality of those structures. MRI may be the next step, but wait and see what the ortho doc says.
