Does subchondral insufficiency fracture of the superolateral aspect of the left femoral head with extensive bone marrow edema require a surgery?

Likely, eventually! It sounds like you probably have some avascular necrosis with subchondral collapse. Unfortunately, the controversial treatments don't usually work once collapse has started. You're likely, eventually, going to need to have a hip replacement. You should see a hip specialist to go over your options at this point.
