4yo vomited and had tummy pain with fever and vomiting then 23hrs fever and vomit free then puked first thing this morning, could this be brain tumor?

Very unlikely. Brain tumor patients puke in the middle of the night after a period of shallow breathing increases their brain pressure. This kid likely just had more appetite than the stomach was ready to handle after a stomach virus.A gradual return to feeding is always best.

Not Likely. Bacterial infections of the throat, usually strep throat, should be responsive to antibiotics. Resistance is less of a worry for strep than other bacteria. I would be concerned about other infections such as urinary tract infections, mononucleosis, and gastroenteritis.
