In brief: It's possible

for an ovary to not be visualized if it is positioned behind bowel loops.
The ultrasound beam does not penetrate through bowel gas. If the ovary cannot be seen on transvaginal ultrasound, it can sometimes be successfully visualized on transabdominal ultrasound. If that fails, pelvic MRI is also useful.
