In brief: See a dentist

In some cases, chronic sinusitis may be caused by a tooth infection.
See your dentist to rule out any dental problem.

In brief: Contact ENT

I would suggest you contact your ENT, for the correct diagnosis and medication to possibly shorten the duration of your illness.
It is not uncommon for upper teeth to hurt with a sinus infection, due to the proximity of the roots to the sinus.

In brief: Sinusitis

Dental and peridontal disease, with periapical abscess formation can produce maxillary sinusitis and should be suspected and investigated.
On the other hand, since the floor of the maxillary sinuses abuts the roots of some teeth this may produce pain. Whether this requires antibiotics should be assessed by the ENT or dentist that you see.
