Location more important. Fibroids along uterine lining (submucosal) often cause heavy bleeding (particularly those 4.5cm). Fibroids on outer part of uterus (subserosal) often cause bulk-related symptoms (urinary frequency, pelvic pain) by pressing on adjacent structures. In between those (intramural) may or may not cause symptoms. If so,see fibroid specialist
