My girlfriend has pain from her shoulder down to her left hand, no numbness, no chest pain she is 53. What can it be?

In brief: Could be cardiac

It could be many things, among them is angina from her heart.
The crux of the decision to present emergently for chest pain should be based off the pre-test probability of how likely it is that her discomfort is from her heart. It depends on her risk factors really. It's less to be cardiac but is still possibly. Musculoskeletal is other possibility, or pinched nerve.

In brief: Could be heart pain

sorting out chest/arm/neck/throat pains folks have can be difficult.
if she is having ongoing pain, she probably needs to be seen as soon as you can; if it is gone and she feels ok, i'd still interview with a physician in the near future.
