Usually muscular. If only of short duration & only occuring with certain positions or movement, it sounds muscular & should resolve with time. You can try over the counter medication and heat to the area like a heating pad. If it persists or worsens or if you develop any other symptoms, see your doctor. Occasionally it can be from other regions like a stomach ulcer or from an actual tumor or spine infection.

Pain to touch? This is usually the sign of a bruise, spasm, or tendinitis. Pain to the touch is not likely nerve related. If the pain is between the spine and your shoulder blade, i would consider levator syndrome (muscle irritation) likely due to posture especially computer work. Start some pain meds. Lie aleve (naproxen) or Ibuprofen (if no allergies or side effects with their use). Using a heat pad would be helpful.
