In brief: Before biopsy
Most of these cases can be managed safely with elimination of caffeine completely, keeping the area dry using corn starch powder regularly, avoiding beer, avoiding any soap directly on the bottom other than what washes off of you during a shower, not rubbing the area, using moist wipes such as cottonelle.
Try to avoid creams especially steroid, they may help but can make things worse if used chron.

In brief: Chronic inflammation
Many different things can cause itching.
The perianal skin is especially sensitive. Common causes of itchiness include aggressive cleaning of the perianal skin, dairy products, caffeine containing foods and certain fruits. The common diagnosis is pruritis ani. Some diseases such as lichen sclerosis can also cause itching. Biopsies help determine if there is an underlying cause of the itching.
