In brief: May not be connected
Many types of chronic kidney disease (that would lead to kidney failure + the need for a txp) do not cause pain, including hypertension, diabetes, + glomerulonephritis.
Others, such as polycystic kidney disease (they became enlarged and filled with cysts - giant blisters) + kidney stones may indeed cause pain. Persistent pain should always be evaluated by an md - disclose the family history.

It also depends on where your back is hurting you. I would go to your doc for blood work to routinely check on kidney function, given that family history. Have them see if your back pain is consistent with renal issues or more typical low back issues. May need ultrasound, mris of kidneys vs spine to determine underlying reasons.
