In brief: Needs evaluation
This could be a hydrocele or possibly a mass if it is fairly painless.
If it is red or inflamed it could be an infection. If it hurts terribly go to er as it could be torsion (twisted). Either way you need evaluation by urology and an ultrasound.

Having one testicle larger than the other is normal. Much larger is worth checking.
