Sounds like you need to have things checked out.
Testicular torsion (a serious problem affecting a testicle that causes blockage in blood flow) can cause severe pain and would need to be evaluated right away. An infection can also cause redness and pain. Some infections (such as yeast in the groin) are less serious and treated with creams; others (such as bacterial) would need oral antibiotics.

But it is not uncommon for even toddlers to have a rash in the diaper area. It could be a yeast rash or bacterial rash. It could be skin irritation. If it is not too bad or painful, try an over the counter barrier cream and/or antifungal such as clotrimazole. See your doctor if not improving.

Sometimes it's just from being in a wet diaper . Change his diaper frequently to avoid redness, if tenderness continues he may need medication to clear it.
