In brief: See psychiatrist

You have a cluster of psychiatric symptoms that will require professional help for management.
This requires an in-depth evaluation with a psychiatrist, and not something suitable for a forum such as this.

In brief: Too many to choose!

There are any number of good medications for all four conditions you identified.
Some medications can treat depression and anxiety, both, with just one med. Concentration can go down if you are depressed, so treating depression might improve that. Otoh, add and fragmented attn can lead to depression & anxiety. A good rule of thumb is to start with the main problem, tackle that, then add rx.
