Does urine can enters into epididymis everytime I urinate? I feel heaviness, pain and tenderness in testis after urination.

Epididymitis. This is not well understood but urine can irritate the prostatic and ejaculatory ducts and cause a reaction of the tissues that can be referred to the epididymis. You may want to see a urologist to rule out possible etiologies of this pain.
