The most common cause i see is that people are ineffective at oral hygiene.
Most don't clean the most important sites--in between the teeth. Also, when they brush, theybrush their teeth instead of removing the plaque from the gumline--the junction of the teeth and gums. Have a good dental check-up and have a hygienist demonstrate proper techniques.
