What symptoms do you look for to screen for acid reflux versus delayed gastric emptying in an infant? How does one know when to refer to specialist?

In brief: Timing of vomiting

Many children spit up, some a lot.
The time after feeding is key for deciding if the stomach has delayed emptying. Infants should have an empty stomach by 2 hours or so after eating, so if a child throws up 4 hrs or more after eating, they likely have delayed emptying. Poor weight gain, poor appetite and cough are all reasons for a referral. Talk to your doctor if you are concerned.
